Outbreaks of mucomycosis (black fungus) have also been reported in other cities across the country.
Spokesperson of the Ministry of Health and Population Dr. Krishna Poudel confirms that this infection started from Birgunj and was seen in Banke, Kathmandu and other cities.
A 67-year-old man was infected with black fungus in Birgunj in the third week of April. But now 12 corona infections have been confirmed in the ministry, he said.
‘The infection that appeared in Birgunj is now also seen in hospitals in Kathmandu,’ said Dr. Poudel said, “We have received information that three people have been infected at TRI Teaching Hospital, two at Veer Hospital and one at Norvik Hospital.” The infection has also been reported at Bheri Hospital in Banke.
While testing three people in Birgunj, two people have been confirmed to have black fungus. The report of one of them is yet to come. Niraj Singh said.
Similarly, three people were found infected with black fungus at Bheri Hospital in Banke. Rajan Pandey said. “Three corona infected people had symptoms similar to those of black fungus,” said Dr Pandey. “Two died without treatment. One of them was referred to Kathmandu.
Spokesperson of the Ministry of Health Dr. According to Krishna Poudel, two / three cases of black fungus have been reported recently. This indicates that there is a strong possibility of black fungus infection in the coming days, he said.
As there is no test for fungus in the hospital, there was no formal confirmation. Pandey said.
Black fungus infection was found in more than 9,000 people who had recovered from corona infection in cities including Maharashtra, Delhi and Gujarat in India.
Black fungal infection has been seen in the patients who have recovered from Covid-19 in Nepal as well. ‘The rate of covid infection is increasing now. Many people have used steroids and antibiotics during the infection, ”said a spokesman for the health ministry. “Black fungus infections are on the rise because of the weakened immune system of people who have covid,” Poudel said.
Dr. According to Poudel, two / three cases of black mold have been reported recently. This indicates that there is a strong possibility of black fungus infection in the coming days, he said.
Patients with severe covidosis who have taken steroids and antibiotics are more likely to develop black fungal infections once they are free of the infection, he said.
Dr. Yogendra Neupane, general secretary of the Association of Nephrologists and Ear and Throat Specialist at the Teaching Hospital, Maharajgunj, estimates that the number of infections will increase from 100 to 150 within a month or two.
Chief Executive Officer of Tilganga Hospital in Kathmandu and cornea specialist Dr. Rita Gurung says it is a rare disease. She said that not only Kovid’s patients but also other patients with weakened immune system get infected.
How is the test done?
Especially patients with diabetes, cancer, kidney, steroid and antibiotic overuse have reduced immunity to the disease. These people have an ‘apertunisky infection’.
As a result, such people have symptoms such as swelling of the eyes, pain, blurred vision, bleeding from the eyes, sudden rise in sugar, and difficulty in breathing. If such symptoms appear, the infection will be tested according to the body parts. Poudel said.
He said that the disease can be identified by mounting the swab of the infected person and performing microscopic test. Similarly, the disease can be identified through culture test and biopsy test. Poudel says.
He said that microscopic test and biopsy test can also be done in labs outside Kathmandu. But culture tests are only possible in some hospitals in Kathmandu, he said.
How to avoid it?
Black mold is a type of ‘Apertuniski’ mold found naturally in our environment. The higher the humidity, the higher the risk of fungal infection. So Dr. Poudel suggests not using wet or cold masks. The habit of using the same mask over and over again should be abandoned immediately. Poudel says.
He also suggested that people who have been using steroids for a long time should control and take them only as per the advice of the doctor. Dr. should discourage the habit of taking medicine according to his will. Poudel said.
Doctors say that antifungal drugs should be taken to prevent this disease. You may need to take medicine for a few weeks or months depending on the area of the infected body, but only on the advice of a doctor.
Cornea specialist Dr. Gurung says that there will be black mold in the environment around us and that it will not attack anyone. During the Covid epidemic, critically ill patients are more likely to become infected when they take steroids to fight off the disease.
She argues that the problem is more prevalent in patients with diabetes and cancer. “You have to be careful not to let your body’s immune system weaken. Don’t use steroid drugs on your own, ‘said Dr. Gurung said, “If you have to use the medicine, you should follow the doctor’s advice.”
What is the treatment method?
Amphotericin B is mainly used in the treatment of people infected with black fungus. Spokesperson of the Ministry of Health Dr. According to Poudel, two / three antifungals are used by people infected with black fungus.
According to him, mainly amphotericin B and posaconazole are used. Among them, amphotericin B Kalazar patients also use the drug.
However, due to the recent decline in the number of kala-azar patients in Nepal, the import of this drug is low and India has banned its export. There is a government analysis that the export of kala-azar medicine may have been banned in India due to the increase in the infection of black fungus.
Ear, Nose and Throat Specialist Dr. Neupane also says that the infection of black fungus will be diagnosed through antifungals and surgery.
The body parts that have been infected with the fungus should be identified, operated on and scraped off, as well as antifungals. Neupane has an argument. He says about 50 percent of infected people die if they are not treated in time.
What is the government doing?
As the incidence of black fungus has been increasing in recent times, the government is moving towards procuring medicines. A spokesman for the Ministry of Health, who has been coordinating with the WHO to bring antifungal drugs, said. Poudel said. “The process of bringing the medicine from the WHO system has already started,” he said. “The medicine may arrive in Nepal by June 3 or 5.”
He said that the arrival of medicine was delayed due to lack of regular flights.
According to him, the health service department was asked to provide 10,000 vials. But WHO is providing 3000 vials in the first lot. He said, ‘A thousand vials are on a regular schedule for Kalajar patients. When 3,000 additional ones come, we will get some relief for now. ‘
What is a protocol?
Dr. According to Poudel, doctors diagnose the infection. Based on the patient’s diagnosis, physicians identify the site of infection and decide on medication. Physicians will study the condition of the patients being treated in the hospital and move the treatment process forward. Poudel says.
Advisory issued for prevention
Dr. Yogendra Neupane, General Secretary of the Association of Ear, Nose and Throat Specialists of the Teaching Hospital, Maharajgunj, told Online Khabar that the Association of Specialist Physicians of Nose, Ear and Throat has already issued advisory on its prevention and management in coordination with the Ministry.
“What is black mold?” What are the symptoms? What are the symptoms of the infection? How to identify it ?, What can be done to adopt awareness ?, How to avoid it ?, What are the treatment methods? An advisory was issued to suggest to the health workers about the medicines to be used.
